Root cause

A reporting module added to the ERP system in March 2026 introduced queries against a table with no index on the date column used in WHERE clauses. Under low load the queries ran acceptably. As report volume increased through April, query times degraded progressively. By 11 April the cumulative load pushed MySQL response times above the Nagios alert threshold.

Resolution

Index added on report_date column of wp_reports table. Query execution time dropped from 7.8 seconds to 0.04 seconds. Nagios threshold test passed. No data loss. No service restart required.

Prevention action

Slow query log enabled on this server going forward. Any query exceeding 2 seconds will be logged and reviewed on the next maintenance cycle. New module deployments to be flagged to AV Services in advance for pre-deployment query review.
